Participating Insurance Plans
Please note that we participate in Medicare, and most third-party insurance plans (with the exception of Jai, Diamond Plan and Kaiser), including those listed on the following page: Participating Insurance Plans.

Preparing for Your First Visit
All patients are required to complete the following forms prior to receiving medical care. New patients are encouraged to print out these forms prior to their first visit, or to arrive 15 minutes early to fill them out.

Please also bring the items below to your first appointment, along with Photo ID.

What to Bring to Your Appointment
To ensure the best quality of care, please come to your appointment with the following items:

  • Your current medications
  • Your insurance card, if relevant
  • Your co-pay, if relevant
  • The Name and Address of your pharmacy. Most prescriptions are sent electronically to the pharmacy which you will give us during check-in.
  • The Name and Phone Number of any other medical provider which you have visited so that we can get your records.

Making an Appointment
To schedule a new appointment, or to cancel or reschedule an existing appointment, please call your office which you have visited. If you are unsure which office you regularly visit, or are a new patient, please call the main office at 410.358.6450 .

Same Day Sick Visits
If you are having a true medical emergency, please call 911 or visit the Emergency Room. If you have an urgent medical need and have visited our practice before, please call our offices as early as possible and all efforts will be made to schedule you for the same day.

Getting Us Your Medical Records
To ensure continuity of care, please let us know if you have received medical care from another provider and request that they FAX their records over to us . You may also point them to http://salujamedical.com/hcp for instructions on sending over records. This includes visits to specialists, urgent care centers and Emergency Rooms, along with testing such as blood work, XRays, Mammograms, CAT Scans and so on. If you have any doubt, please ask anyway.

Prescription Refills
Prescription refills should be requested by your pharmacy. To be eligible, you must have visited us within the last 60 days.

Referral Requests
Referrals may be requested directly by the facility you are visiting, or by calling our offices. To be eligible for a referral, you must have visited us within the last 60 days.
